Its a little hard to see the actual configuration of your actuator without some more pics, but Progressive Automations has a pretty complete line of linear actuators that can satisfy most applications.

I don't know if this helps. That's from a airplane, Lear manufacture, maybe for ailerons or?
Can you get better model number pics? Maybe start calling airplane repair shops to get fixed or bone yards. They do have new ones, but not like that exact for flaps. I did a quick search and didn't find anything.I did figure out that it’s from an airplane and Lear made parts for planes. But I can’t seem to find much info about Lear or if these are still being made. I was hoping to find the same one that I have to make it easy to install and not have to change the mounting bracket.
